List of Required and Supplemental Documents
Required:
- Birth certificate / Original household registration
- Current household registration
- Marriage certificate/Affidavit of Marital Status, if applicable
- Divorce/Death certificate , if applicable
- Police certificate from your country of origin if you have lived there for more than 6 months after the age of 16 and any other country in which you have lived for more than 1 year. This certificate must have been issued within one year prior to applying for your visa. A police certificate is valid indefinitely if it was issued by a foreign country that you never visited again after the issuance of the police certificate.
- I-864/I-864A from petitioner and joint sponsor (if applicable)
- U.S. tax return, Form 1040/W-2 for petitioner and any joint sponsors, for the most recent tax year
- Medical Report (performed within past 6 months)
- Your current and expired passports
Supplemental Documents (needed in some cases):
If you have a court record:
- Court record and English translation
In some cases, you need to provide more proof of the sponsor’s domicile in the U.S.
- Proof of the sponsor’s domicile in the U.S.
In some cases, the applicant may need to bring:
- Taiwan Entry/Exit record for past ___ years
If petitioner/joint sponsor did not file U.S. tax return:
- A statement explaining the reason for not filing taxes in the U.S. for the most recent three year
